Poly pharmacy and medicines optimisation

In a recent publication of The King’s Fund the poly pharmacy and medicines optimisation has been addressed. The authors define poly pharmacy as situation where people are prescribed multiple medications where it can be either appropriate or problematic. Increasing levels of multi-morbidity causes clinicians to use multi medications, but there appears to be a lack of guidelines how and when to use them. The paper summarizes that medication regiments have to be managed well, or patients will not take their medication resulting in costly waste and failure of treatment.

We can only confirm the importance to empower patients so they understand the ‘why’ of their treatment. We also understand that the poly pharmacy patient needs a system to ease the minds and supports the regiment adherence. It’s good to know that this kind of support already exists!
