Patient activation for less readmission

Medicatie AdherenceHaving the knowledge, skills and confidence to manage one’s health – called patient activation – leads to lower readmission rates, according to researchers from the Boston Medical Center. This study, with 695 subjects, was setup to research the role of patient activation in the rate of hospital readmission within 30 days of the hospital discharge. The conclusion is that hospitalized patients with lower levels of patient activation had a higher hospital readmission rate. So, we need to activate patients!
Hospitals can do this in many different ways, like supporting patients in their medication adherence. Support your patient’s medication adherence with the PICO and activate them for lower readmission rates.
